Our Practice is open for registration to anyone eligible for NHS care residing in the SS8 area. You can register by completing the necessary form in person at our Practice or by emailing us at oaklands.surgery@nhs.net. When registering, it’s ideal to have your

- Photo ID
- Proof of Address
